Kava and Liver Safety: Myth vs. What the Research Actually Found

Is kava bad for your liver? The short answer: the liver scare of the early 2000s was real, but it was tied to a specific kind of product — solvent-extracted pills, often made from the wrong parts of the plant. Traditionally prepared noble kava root has a very different safety record. Here is what the evidence actually shows.

Where the Kava Liver Damage Scare Came From

Starting in the late 1990s, reports of serious liver injury in people taking kava supplements began appearing in Europe. By the end of 2002, Germany’s Federal Institute for Drugs and Medical Devices (BfArM) had collected around 40 case reports, a handful involving liver transplants or deaths. BfArM withdrew kava’s marketing authorizations in 2002, and restrictions followed in Switzerland, the UK, Canada, Australia and elsewhere. The US FDA issued a consumer advisory the same year.

The reaction was understandable. What was not asked loudly enough at the time was what kind of kava these people had actually been taking.

Almost without exception, the answer was concentrated acetone or ethanol extracts sold as pills and capsules — not the water-based root drink that Pacific Island cultures have used for roughly 3,000 years.

What the World Health Organization Found

In 2007 the World Health Organization published Assessment of the Risk of Hepatotoxicity with Kava Products, an independent expert review of every available case report, clinical trial and safety study. It remains the most thorough look at kava and liver safety ever conducted.

The committee reviewed 93 case reports from regulators in Germany, Switzerland, the US, the UK and other countries:

  • Most reports were incomplete or could not be assessed because key clinical information was missing.
  • Only 8 cases were rated as having a “probable” link to kava.
  • Most patients had used acetone or ethanol extract products, not traditional water-prepared kava.
  • Many involved pre-existing liver disease, heavy alcohol use, or other liver-stressing medications.

The committee drew a clear line between the two kinds of product: water extracts as consumed in the South Pacific, with hardly any serious liver problems reported, are chemically different from the solvent extracts used to make kava pills in Europe — and that difference could explain the liver injuries seen in some pill users.

Why Traditional Kava and Kava Pills Are Not the Same Thing

The active compounds in kava root are kavalactones. How you get them out of the root changes what else comes along with them.

  • Traditional preparation kneads ground root in water. The resulting drink carries kavalactones in a natural suspension along with other water-soluble root compounds. One proposed explanation for kava’s traditional safety record is that water preparations retain glutathione, an antioxidant involved in how the liver processes kavalactones, which solvent extraction does not carry over.
  • Solvent extraction with acetone or ethanol concentrates kavalactones efficiently, but also pulls out compounds that water largely leaves behind, including flavokavains (a class of chalcones that have shown liver-cell toxicity in laboratory studies).
  • Plant part matters. Some European manufacturers used cheaper stem peelings and above-ground plant parts instead of root. These contain the alkaloid pipermethystine, which has been proposed as a contributor to the liver cases. Traditional kava uses peeled root only.
  • Raw material quality. A 2012 review by Teschke and colleagues, revisiting the “Pacific kava paradox” (Pacific drinkers without liver problems, European pill users with them), concluded that poor-quality raw material was the most likely culprit — including non-noble cultivars and root contaminated by mold during improper drying or storage.

That is why the WHO recommended that kava products use only the peeled root and rhizome of Piper methysticum, exclude stem peelings and aerial parts, and favor water-based preparations. It is also why noble kava cultivars — the traditional drinking varieties — matter.

3,000 Years of Traditional Use

Kava has been drunk ceremonially and socially across Fiji, Vanuatu, Samoa, Tonga and Hawaii for millennia, and liver damage was never a recognized concern in those communities.

A 2004 pilot study in Fiji, cited in the WHO report, examined heavy traditional drinkers who had consumed an estimated 100,000 bowls of kava over their lifetimes and found no association between kava drinking and liver disease. A separate survey of traditional healers and medical practitioners in Samoa found no reports of the classic signs of liver trouble — yellowing eyes, dark urine — linked to kava drinking.

What Clinical Trials Show

The WHO committee’s conclusion on controlled research was direct: clinical trials of kava have not revealed liver toxicity.

  • A Cochrane review of 11 randomized trials (Pittler & Ernst) found kava extract effective for anxiety and relatively free of side effects over short-term use.
  • Lehrl’s 2004 trial (61 patients, four weeks) and the 2009 KADSS trial led by Sarris (60 patients using a water-soluble kava extract) both reported normal liver function throughout.
  • Where minor liver enzyme elevations were seen in trials, they were small and returned to normal after kava was stopped.

How Regulators Have Changed Course

The bans did not age well. In 2014 a German administrative court ruled that BfArM’s 2002 withdrawal of kava was not justified by the evidence, and that ruling was upheld on appeal in 2015. Other countries have since relaxed restrictions, and international food standards for traditional kava beverages now exist — a long way from the blanket bans of the early 2000s.

Kava Side Effects Worth Knowing

Liver injury is rare and tied to specific risk factors. The side effects people actually notice are usually mild:

  • Numb mouth and tongue for a few minutes after drinking — normal and harmless.
  • Mild loss of coordination or drowsiness at higher doses. Do not drive after drinking kava.
  • Upset stomach in some people, especially on an empty stomach.
  • Kava dermopathy — dry, scaly skin after heavy, prolonged daily use. It reverses when you cut back or take a break.

Who Should Be Careful With Kava

For the rare cases where a kava link was plausible, the WHO identified clear risk factors. Be cautious, and talk to your doctor first, if any of these apply:

  • You have existing liver disease or a history of hepatitis.
  • You drink alcohol heavily, or plan to combine kava with alcohol.
  • You take medications processed by the liver or that act on the central nervous system — including benzodiazepines and other sedatives, anti-anxiety medications, antipsychotics, anticonvulsants, blood thinners, or levodopa.
  • You are considering concentrated extract products of unknown origin, or doses well above label recommendations.
  • You are pregnant or breastfeeding.

For healthy adults, sensible use looks like this: choose noble kava root, prepare it traditionally or choose reputable products that disclose their source and extraction method, start with a modest amount (see our kava dosage guide), and do not mix kava with alcohol.

Kava and Liver Safety: Common Questions

Does kava cause liver damage?

Serious liver injury linked to kava is rare. The WHO’s 2007 review found only 8 of 93 reported cases had a probable link to kava, mostly involving solvent-extracted pills, pre-existing liver disease, alcohol, or other medications. Clinical trials have not shown liver toxicity.

Is the kava liver damage claim a myth?

Partly. The case reports were real, but the evidence behind blanket bans was weak and did not separate traditional water-prepared root from concentrated extracts made from lower-quality plant material. Germany’s own ban was overturned in court in 2014.

Is kava bad for your liver if you drink it every day?

Traditional daily drinking in the Pacific has not been associated with liver disease. Very heavy, prolonged use can cause other effects such as dry skin, and anyone with liver disease or who drinks alcohol heavily should speak with a doctor first.

Are kava capsules safer or riskier than kava drinks?

It depends on what is in them. Capsules filled with whole noble kava root powder are close to traditional kava. Products made from solvent extracts of unknown plant material were the type most often involved in the European liver reports.

Can I drink kava with alcohol?

It is best not to. Combining kava with alcohol increases sedation, and heavy alcohol use was one of the most common factors in the liver cases the WHO reviewed.

It seems that the medical establishment and pharmaceutical companies like Merck and Eli Lily (WeLie Lily) want you to believe that Kava is very dangerous and damaging to the liver etc… They want to deter people from using natural alternatives in order to keep the money flowing in from their synthetic man made junk. Most all of the natural alternatives are equally as effective if not more so and also less dangerous than most of the man made stuff. If you go to WebMd to read about Kava you will find a warning that says, “Kava is POSSIBLY UNSAFE when taken by mouth. Don’t use it. Serious illness, including liver damage, has occurred even with short-term use of normal doses.” But they don’t tell you that the pure un refined root powder doesn’t do that and is safe but everything we consume does effect the liver to some degree. The drug Cymbalta has serious effects on the liver and has caused deaths and the need for liver transplants and they don’t tell you not to use it. And Kava is safer than Cymbalta . It’s all about the money and big pharma trying to snuff out the natural alternatives competition.

  3. I just bought Yogi Kava Tea, and I know it’s no where as strong as powder, that’s how I’d like to start, slow. If it is as harmful as say one cup of wine, that’s fine with me. Especially if I only need one brewed cup!! They should definitely be more descriptive of how harmful it is if they ‘know so much’ about it! It’s like you said everything is harmful at large quantities; grease, pollutions, sleep, water, you name it!

  4. We have a large library of studies that have been conducted on the safety of Kava root. In short, the folks over in Oceania have taken Kava daily for several thousand years, and in much higher quantities and strengths than the FDA currently allows, with virtually no reports of any harmful side effects. Most of the rumors of liver damage are from Kava products made with parts other than the root. More info on Kava’s biotany would make your readers appreciate the plant more if you could tell us more about the life cycle and age of plants at harvest and how they are processed into the products.
